Celebrate Arts Award’s first decade by choosing the best art!

Is there a musician that's appeared in the last ten years who's got you like no one else? A TV show that has had you so gripped you've forgotten to have a social life? A piece of art that sums up life, death and everything in between for you? Well, now is your change to have it honoured!

Voting is very simple on the form below. Just pick your favourite for each art form, or if you don't see it there, type it into the 'other' box at the bottom of each selection. If you really don't know, or if you hate one art form (God forbid!) please just select the 'Don't know!' option and move on to the next. Remember to add your email address at the bottom of the form. We'll enter you into a prize draw to WIN:

A Hudl, £40 Theatre Tokens, an annual under-26 Art Pass or a £20 iTunes Voucher.

NB: Winners have now been selected, using the automatic generator, Random Picker.

If you're struggling to remember a whopping ten years ago, our superb associate editor, Bhavesh Jadva has put together videos for each art form, you can check them out below...
